内科治療不応の低出生体重児の動脈管開存に対する治療は従来外科治療が唯一の治療となっていた．2009年のAmplatzer® Duct Occluderをはじめとし，2020年には特に低出生体重児に有効なデバイスとしてAmplatzer Piccolo® Occluderが導入され，その有効性に期待がもたれている．日本先天性心疾患インターベンション学会が定める「適正使用に関する手引き」ステップ2にあたる手技時1～2 kgの症例を2例経験した．いずれも“Infant Heart Team”の構成により安全かつ有効に治療を行うことができたので報告する．
Surgery has been the primary treatment option for patent ductus arteriosus (PDA) in low-birth-weight infants refractory to medical therapy. The Amplatzer® Duct Occluder was introduced in 2009, and the Amplatzer Piccolo® Occluder was introduced in 2020 as an effective device, especially for low-birth-weight infants. We report 2 cases, who had symptomatic PDA and body weight of 1 to 2 kg at the time of the procedure, successfully treated by Piccolo®, which is step 2 of the “Guide for Appropriate Use” by the Japanese Society for Congenital Interventional Cardiology. Both cases were treated safely and effectively by the “Infant Heart Team”.
